Skin Biopsy Cases

Follow these easy steps to package and chip laboratory samples to University of Utah Dermatopathology Laboratory for processing and testing. Use the Test Submission Checklist for a step-by-step guide:

Packaging

  1. Collect the specimen and place it in the appropriate specimen fixative.

  2. Label specimen vial with patient name, second patient identifier, date of collection and specimen type (corresponding to the designation on the Request Form).

  3. Close tightly each specimen vial to avoid leakage during transportation.

  4. Place the specimen vial inside the biohazard bag.

  5. Complete the University of Utah Dermatopathology Request Form.

  6. Place the completed Request Form in the side pocket of the biohazard bag.

Shipping

  1. Place the biohazard bag inside the FedEx® Clinical Pak. (laboratory shipping bag).

  2. Seal the FedEx Clinical Pak and attach a FedEx pre-labeled and prepaid FedEx air bill.

  3. For FedEx pickup, please call FedEx at (800) 463-3339 or schedule online at www.fedex.com.

Consultation Cases

  1. Please place pathology slides in the appropriate slide container and wrap in bubble wrap.

  2. Ship either our Consultation Request Form or a clinician’s consult request letter.

  3. Please provide us with your complete or incomplete pathology report (including gross description), H&E glass slides, and pertinent special or IHC stains.

  4. Patient historical information and available pictures would be greatly appreciated and will aid us with consultative opinion.

  5. If special studies are anticipated or requested, sending selected paraffin blocks will expedite consultation analysis.

  6. Place all of the appropriate consult material inside the FedEx® Clinical Pak. (laboratory shipping bag).

  7. Seal the FedEx Clinical Pak and attach a FedEx pre-labeled and prepaid FedEx air bill.

Specimen Labeling

Label each specimen bottle/vial with at least 2 identifiers, along with specimen type and body site:

  • Patient’s full name

  • Second identifier

    • Date of birth (DOB) and/or

    • Hospital or clinic number and/or

    • Patient ID number and/or

    • Accession number

    • Specimen type

    • Body Site

Dermatopathology Request Form Instructions

Complete the Dermatopathology Request Form with the patient’s testing and billing information and any other pertinent information. It is very important that each requisition includes the following:

  • Patient’s first and last name

  • Date of birth (DOB)

  • Gender

  • Date and time of collection

  • Ordering Physician’s name and signature

  • Test(s) requested

  • Pertinent clinical findings/history

FedEx Shipping Supplies

  • FedEx Clinical Pak, is a leak-proof envelope required for shipping laboratory samples, which allows packaging of multiple specimens into one shipping envelope.

  • Pre-printed air bills, showing our shipping address and billing account number.
